Personal Health Records Can Improve Health Outcomes

Personal Health Records (PHR) are an electronic tool that are used by patients to store and manage their own health data. The PHR serves as a means for patients to track their health conditions and treatments. PHR also serves as a means for patients to communicate as they see fit with a wide range of clinicians. Used correctly, this tool can improve communications and patient satisfaction by encouraging patients to become an integral part of their health care process. Not only is this a mandatory component of HHS/CMS requirement Meaningful Use 2, but it's a necessary means for improving health outcomes and affecting population health.
